Service Description
If you have been involved in an auto accident and are experiencing pain, stiffness, or limited movement, massage therapy may be covered under your no fault insurance when deemed medically necessary. Early care is important. Addressing soft tissue restrictions sooner after injury can support recovery, reduce compensation patterns and help restore normal movement. There are typically no out-of-pocket cost for treatment when you have proper script/referral from your physician or chiropractor. Those recovering from surgery or injury may find massage helpful in lengthening muscles, breaking scar tissue and myofascial adhesions, which can facilitate faster recovery and mobility. Conditions treated include: -Back pain -Neck pain -Shoulder pain -Muscle pain -Inflammation and muscle spasms -Post-surgical recovery. PLEASE NOTE: Please Note Booking this service is a request for care, not a confirmed appointment. All no-fault cases require: -Verification of insurance benefits -Script or referral -Completion of intake and required paperwork Appointments are only confirmed once verification is completed.
Cancellation Policy
Client Agreement By signing this agreement, you acknowledge that you have read, understood, and agree to the following: Consent to Treatment I voluntarily consent to receive massage therapy, reflexology, herbal wellness services, and other wellness services provided by Fruitful Vibrations LLC. I understand these services are intended to promote relaxation, improve mobility, reduce muscular tension, and support overall wellness. They are not intended to diagnose, prescribe, or replace medical care. I agree to inform my practitioner of any changes in my health, medications, pregnancy, surgeries, or injuries before each visit and will immediately communicate any discomfort during treatment. I understand that temporary soreness or bruising may occur following a session. Financial & Cancellation Policy -Payment is due at the time of service unless otherwise arranged. A non-refundable $25 deposit is required for private-pay appointments when a card is not kept on file. Insurance benefits are verified as a courtesy and are not a guarantee of payment. I am responsible for any copays, deductibles, coinsurance, denied claims, or non-covered services. -I agree to provide at least 24 hours' notice if I need to cancel or reschedule an appointment. Late cancellations and no-call/no-show appointments may be subject to applicable fees. Emergencies and unforeseen circumstances will be reviewed on a case-by-case basis. Privacy & Professional Standards My personal and health information will remain confidential and used only for treatment, scheduling, billing, insurance processing (when applicable), and other business operations permitted by law. I understand that all services are professional, therapeutic, and non-sexual in nature. Inappropriate behavior may result in the immediate termination of the session without a refund. Acknowledgement I certify that the information I have provided is accurate to the best of my knowledge. I have had the opportunity to ask questions and voluntarily consent to receive services from Fruitful Vibrations LLC under the policies outlined above.
